Rivaroxaban for Dogs
Thrombosis represents the leading cause of mortality in canine Immune-mediated Haemolytic Anaemia. Rivaroxaban is a modern, direct oral anticoagulant (DOAC) increasingly utilised by veterinary clinicians to prevent life-threatening pulmonary thromboembolisms during acute haemolytic crises.
How Rivaroxaban Works in Dogs
Rivaroxaban belongs to the class of direct oral anticoagulants (DOACs). Unlike older drugs such as warfarin or heparin, which require careful, frequent blood monitoring, rivaroxaban targets a specific enzyme within the clotting pathway.
It acts as a highly selective and reversible inhibitor of free and clot-bound **Factor Xa**. Factor Xa sits at the crucial junction of the intrinsic and extrinsic coagulation cascades (the common pathway). Its primary job is to convert inactive prothrombin into active thrombin.
By directly blocking Factor Xa, rivaroxaban halts the generation of the "thrombin burst" required to convert soluble fibrinogen into insoluble fibrin strands. This stops the formation of the structural meshwork of a blood clot without disabling platelet aggregation directly.
Direct Anticoagulants vs Antiplatelets
Both clopidogrel (an antiplatelet) and rivaroxaban (an anticoagulant) are excellent first-line options for canine IMHA, but they target different areas of the hypercoagulable state.
Note: The 2019 CURATIVE guidelines recommend selecting one primary antithrombotic as monotherapy. Standard protocols suggest avoiding the routine combination of clopidogrel and rivaroxaban due to a significantly elevated risk of internal bleeding.

Absorption and Meal Timing
Rivaroxaban is well absorbed in the canine gastrointestinal tract. For doses exceeding 0.5 mg/kg, bioavailability is optimised when administered alongside a meal. Always give rivaroxaban with a small amount of food to prevent transient stomach upset.

No Loading Dose Requirements
Unlike clopidogrel, which requires a heavy loading dose to achieve rapid therapeutic platelet blockade, rivaroxaban reaches peak plasma inhibition within 1 to 3 hours of the very first dose. No acute loading protocol is necessary.

Surgical Washout Guidelines
Because rivaroxaban is cleared relatively quickly by renal and hepatic pathways in dogs, stopping therapy 48 to 72 hours prior to an elective surgical procedure is generally sufficient to restore normal haemostasis.
