IMHA Relapse & Safe Tapering
Achieving clinical remission is a massive milestone in managing Immune-mediated Haemolytic Anaemia. However, the path to long-term recovery is closely tied to your medication tapering schedule. Tapering too rapidly represents the single most common cause of sudden autoimmune relapse.
Defining Clinical Remission
Remission is not merely the absence of visible symptoms. From a clinical perspective, true remission requires the complete stabilisation of the patient's red blood cell count alongside the resolution of active autoimmune destruction.
To confirm a patient is ready to begin a drug reduction programme, the packed cell volume (PCV) must remain stable above 30% for a minimum of two to three weeks. In addition, secondary blood smear checks must show that spherocytes have disappeared and slide auto-agglutination has completely resolved.
A relapse occurs when the immune system suddenly restarts its attack on the red blood cells, causing a rapid decline in oxygen-carrying capacity. This is typically triggered by a rapid reduction in immunosuppressive drugs, or by underlying inflammatory events like systemic infections.
Relapse Timelines and Statistics
Autoimmune recurrence rates are well-documented across veterinary medicine. Understanding when a relapse is most likely to occur allows clinical teams to schedule monitoring appointments proactively.
Canine Relapse Rates
A major 24-month multicentre study published in the Journal of Veterinary Internal Medicine by Sparrow et al. documented that the canine IMHA relapse rate is 11% at 12 months, rising to 18% by 24 months. Over half of all documented relapses clustered within the first 90 days of starting the steroid reduction programme.
Feline Relapse Rates
Feline studies report relapse rates ranging between 11% and 15%. Cats are highly sensitive to sudden dosage changes. Relapses in cats are frequently linked to the premature withdrawal of secondary immunomodulators, or the flare-up of hidden secondary viral or parasitic infections.
Equine Relapse Variables
Adult equine patients experience guarded outcomes. Maintaining a very gradual, highly monitored steroid reduction over several months is required to prevent a return of haemolytic anaemia while protecting the horse from steroid-induced laminitis.

Taper Step-by-Step
Reduce the daily prednisolone dose by no more than 25% to 50% at each step. Never double-taper or make massive single-step dose cuts.

Maintain Stable Intervals
Allow a minimum of 3 to 4 weeks between each dosage reduction. This gives the immune system ample time to adjust to the lower level of suppression.

Verify via Serial Blood Counts
Perform a complete packed cell volume (PCV) check 7 to 10 days after every single dose reduction. Never reduce a dose without validating that the haematocrit remains stable.

The Multi-Drug Sequential Rule
If the patient is on dual therapy, always taper the steroid completely first. Keep the secondary agent (like Mycophenolate) at its full active dose to protect the patient. Only begin tapering the secondary agent once the steroid is completely discontinued.

🚨 Spotting Relapse Warning Signs
Veterinary teams and owners must monitor the patient daily for any of these signs, which warrant an immediate veterinary consultation and a priority PCV check:
- • Pale, white, or yellow mucous membranes (gums)
- • Sudden return of lethargy or profound weakness
- • Tea-coloured or reddish urine (haemoglobinuria)
- • Sleeping breathing rate rising consistently above 30
- • Loss of appetite or refusal of meals
