Prednisolone Side Effects in Dogs
Corticosteroids are the absolute cornerstone of canine IMHA therapy. However, the high immunosuppressive doses required to halt the destruction of red blood cells will alter your dog's physiology and behaviour profoundly. Managing these side effects is a critical part of the long-term recovery programme.
Acute Phase Side Effects (Weeks 1-4)
These physiological changes occur almost immediately after commencing high-dose prednisolone induction. They are direct pharmacological consequences of the drug and cannot be avoided.
Prednisolone directly inhibits antidiuretic hormone (ADH) in the kidneys, stopping the body from concentrating urine. Dogs will experience an urgent, frequent need to urinate massive volumes of clear fluid, driving them to drink water constantly. Never restrict their access to fresh water, as this can lead to rapid, dangerous dehydration.
The steroid acts as a powerful stimulant on the appetite centre of the brain. Dogs will beg constantly, steal food, or search through bins. Do not increase their primary meal portions, as this will lead to unhealthy weight gain and put extra strain on their joints.
Prednisolone causes mild weakness in the respiratory muscles and directly stimulates the respiratory centre in the brain. This results in continuous, dry, open-mouthed panting, even when the dog is resting in a cool room.
Chronic Phase Side Effects (Month 2+)
As the steroid therapy continues over several months, the catabolic (tissue breakdown) actions of the drug begin to manifest physically.
The steroid breaks down proteins in the skeletal muscles to fuel liver glucose production. This causes prominent wasting over the skull (temporalis muscles), spine, and hindquarters, making the dog look frail and struggle with steps.
A combination of progressive abdominal muscle weakness and steroid-induced fat redistribution causes the stomach to sag noticeably. This is often exacerbated by liver enlargement (hepatomegaly) caused by glycogen accumulation.
The skin loses its thickness, becoming paper-thin and easily bruised. In some cases, calcium deposits build up within the dermis (calcinosis cutis), appearing as hard, gritty, red, or white plaques that are intensely itchy.
Critical Clinical Hazards
Certain side effects represent serious clinical risks that require prompt veterinary screening and protective medical protocols.
Silent Urinary Tract Infections (UTIs)
Because prednisolone suppresses the inflammatory response, a dog with a severe bladder infection will not display typical symptoms like straining, painful urination, or blood in the urine. These "silent" UTIs are common, occurring in up to 30% of dogs on long-term steroids. Routine urine cultures, rather than simple urinalysis, are mandatory during the tapering phase.
Gastrointestinal Ulceration & NSAID Contraindication
Corticosteroids decrease the production of protective prostaglandins in the stomach lining, increasing the risk of deep ulcers. You must never combine prednisolone with NSAIDs (non-steroidal anti-inflammatory drugs like Meloxicam, Carprofen, or Loxicom). Combining these classes is highly likely to cause catastrophic, fatal gastrointestinal perforation.

⚠️ Safe Home Management Rules
- • Free access to water: Always provide multiple water bowls. Restricting water when PU/PD is active can trigger severe kidney stress.
- • Prevent Accidents: Take the dog out for toilet breaks every 2 to 3 hours. Do not punish them for accidents inside the house.
- • Do not Taper Alone: Never reduce or stop prednisolone without direct instructions and a verified PCV reading from your veterinary surgeon.
