IMHA Survival Rates & Prognosis

Understanding the clinical survival metrics for Immune-mediated Haemolytic Anaemia is vital for veterinary surgeons managing cases and owners navigating recovery. Prognosis varies significantly depending on species, rapid drug response, and co-existing clotting complications.

Canine IMHA Survival Metrics

Immune-mediated haemolytic anaemia in dogs remains a guarded and serious condition. Historically, canine fatality was reported to be high, ranging between 29% and 70%.

With modern antithrombotic therapies like Clopidogrel or Rivaroxaban, acute survival-to-discharge has improved significantly. Current consensus literature indicates that between 50% and 80% of dogs survive the acute hospitalisation phase.

However, long-term survival remains challenging. The overall one-year survival rate is between 50% and 60%. This drop-off is driven primarily by disease relapses during steroid tapering, or delayed thrombotic complications like pulmonary thromboembolism.

Key Prognostic Risk Indicators:
  • Severe, persistent auto-agglutination
  • Extreme hyperbilirubinaemia (severe icterus or yellow tissues)
  • Profound thrombocytopenia (co-existing Evans Syndrome)
  • Non-regenerative bone marrow responses

Feline IMHA Survival Metrics

IMHA in cats is more frequently secondary to underlying triggers than in dogs. Prognosis depends heavily on identifying and treating the root cause of the haemolysis.

Retrospective studies show feline survival-to-discharge rates are approximately 60% to 75%. If the anaemia is primary or caused by treatable infectious agents like Mycoplasma haemofelis, long-term remission is highly achievable with prednisolone and appropriate antibiotics.

Conversely, if the feline patient tests positive for retroviruses like FeLV or FIV, or if the haemolysis is triggered by underlying cancer, the long-term survival rate is very low.

Key Prognostic Risk Indicators:
  • FeLV or FIV positive screening status
  • Acute, severe non-regenerative presentation
  • Severe renal or hepatic co-morbidities

Equine IMHA Survival Metrics

IMHA in adult equids is rare, but presents severe clinical management challenges. In contrast, Neonatal Isoerythrolysis in newborn foals represents a common and distinct form of the disease.

A major retrospective equid study published in the Equine Veterinary Journal evaluated 25 equids with immune-mediated haematic disorders. Survival to discharge for these horses was reported at 56%.

For newborn foals suffering from Neonatal Isoerythrolysis, survival rates are much higher, ranging between 75% and 90%. This excellent outcome depends on immediate restriction of colostrum intake, supportive care, and rapid access to compatible blood transfusions.

  • Development of corticosteroid-induced Laminitis
  • Triggering factors like internal abscesses or cancer
  • Profound hypoxia leading to recumbency
